Description
Mr. Mark was see today while in the community. He was alert and oriented, denied skin cut or bruise. Client verbalized he was well behaved while in the community. Vital sign was completed while blood pressure was 148/83, Pulse 89, resp 19, O2 100 and client denied pain and discomfort. Nursing teaching was completed on management of high blood pressure.
